Action
ACTION:
1. Approve the submission of grant application to US Department of Health and Human Services, Office of Minority Health up to $2,400,000 in the General Grant Fund (G001) for the period of August 1, 2024 - July 31, 2028. The funds will be used to improve health outcomes.
2. If awarded recognize, receive and appropriate the award amount to the General Grant Fund (G001) in Public Health.
3. If awarded create 4 full-time positions for this program in the General Grant Fund (G001) in Public Health.
4. Adopt the required grant project ordinance for US Department of Health and Human Services, Office of Minority Health grant in the General Grant Fund (G001).

BACKGROUND/JUSTIFICATION:
Public Health is applying for The Community Level Innovations for Improving Health Outcomes grant. This grant opportunity seeks to increase the use of preventative health services, improve health outcomes as measured by progress toward Healthy People 2030, and reduce health disparities. Public Health proposes to engage two community health workers, a supervisor, and a data analyst at the Ella B. Scarborough CRC to improve healthcare access and health outcomes.
