ACTION:
1) Approve the application to NC DHHS, Division of Public Health Women, Infant, and Community Wellness Section for a three-year period (June 1, 2027 to May 31, 2030) for Supporting Women's Health Services up to $450,000 to the General fund (0001).
2) If awarded, create one (1) full-time position in the General fund (0001) within Public Health.

BACKGROUND/JUSTIFICATION:
The Mecklenburg County Health Department is requesting approval to apply for funding from the NC DHHS, Division of Public Health for Women's Health Services funds to integrate a Community Health Worker (CHW) Model into the Maternal Health Services. The funds will be used to improve maternal and infant health outcomes through outreach, education, care coordination, resource navigation and connections to community-based services.
